Care And Quality Coordinator, Career Opportunity in Toronto, Ontario

Job description

The responsibilities of the role include but are not limited to:

Restorative Care

  • Assess, select and discharge residents from the programs
  • Document all necessary details of the Restorative programs – as per compliance guidelines
  • Organize the delivery of the programs, and
  • Deliver education, amongst others

Skin and Wound

  • Monitor the efficiency of the program as per residents’ care needs and progress
  • Complete necessary documentation (ie: workbook), and
  • Deliver education, amongst others
  • All in collaboration with the Wound Care nurse and Nursing team overall

Pain and Palliative lead

  • Assess and refer individual case referrals to appropriate resources
  • Follow up with Palliative Care checklists – ie: completion, effectiveness as per residents’ individual needs
  • Document all necessary details of the programs – as per compliance guidelines
  • Deliver education, amongst others

Fall Prevention and Restraint/PASD Co. –

  • Recommend interventions for individual residents based on their individual risk and history of falls
  • Analyze the effectiveness and progress of the program for the individual residents’ needs, and suggest/implement interventions for improvement
  • Document all necessary details of the programs – as per compliance guidelines
  • Deliver education, amongst others

Continuous Quality Improvement

  • Participate in the carry out of audits, and suggest/implement interventions for improvement
  • Implement Quality Improvement Protocols guidelines, record their analysis and suggest/implement interventions for improvement
  • Complete Annual program evaluation for the dedicated clinical programs

Other duties include, but are not limited to:

  • Provide back up for MDS
  • Assist with assessments of new residents and facilitate their transition to the Long-Term Care
  • Assist with care and services during Dr’s rounds
  • Assist with recognizing, communicating the needs of the residents to the PSWs and integrating into their care plan/program
  • Responds to resident or family member concerns and ensures appropriate action is taken within decision-making authority and/or brings to the attention of the Director of Care/Administrator
  • Participates in identifying PSW learning needs and makes recommendations for appropriate education
  • Participates in special projects as required
  • Effectively communicates and interacts with residents, family members, visitors and volunteers in a professional and courteous manner

The ideal candidate will possess:

  • All Registered Nurses must have a current Certificate of Competence from the College of Nurses of Ontario and be a member in good standing;
  • Knowledge of the Long Term Care Act, Program Standards for Long Term Care, the appropriate sections of the Health Disciplines Act, Occupation Health and Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada), Coroners Act, College of Nurses of Ontario Standards of Practice and other pertinent Federal, Provincial and Municipal Statues which influence the operation of the Nursing Home and more specifically the nursing department;

 Candidates must be available to work days, evening, nights, and weekends.
