Overview:

The Certified Nurse Assistant (CNA) possesses a sincere passion for working with seniors, as well as have demonstrated knowledge of care giving in a senior living community. This position assumes responsibility for resident care and services including personal needs, grooming and hygiene, housekeeping, laundering, social interactions, and meals. A primary responsibility of this position is to routinely monitor the resident’s comfort and respond to periodic needs.

Responsibilities:

• Provide direct nursing care to residents and contribute to continuity of care and effective communication by reporting care provided and changes in the resident's condition to the Resident Care Director immediately. May participate in routine activities of resident’s care activities, including daily living and basic nursing procedures.
• Assisting residents with lifts, moves and transports, using proper body mechanics or lifting devices for accident prevention.
• Responds to inquiries relating to requests from residents, visitors and other personnel promptly and courteously within given time frames and established policy.
• Is knowledgeable of the individualized care plan for residents, and provides support to the residents according to their care plan. Contributes to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents’ needs and preferences.
• Communicates and interacts effectively and tactfully with the residents, visitors, families, peers and supervisors promoting a homelike environment.
• Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents’ rights.
• Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials. Also providing care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas

Overview:

The Activity Assistant assists the Activity Director in leading and executing the Activities program for the community. The Activities program should encompass activities that require the use of all five senses for the residents. The Activity Assistant carries out a specific set of tasks to fulfill the mission of the Activities program. The Activity Assistant works closely with the Resident Care Director to document any changes in behavior or condition of the residents. 

Responsibilities:

• Assists the Activity Director with planning, developing, and preparing a variety of exercise, social, recreational and educational events.
• Conducts all transportation as well as medical transportation for residents.
• Develops, writes, and circulates the community monthly and weekly newsletter.
• Assists with move-in process by providing information and guidance to new residents.
• Maintains knowledge of communities, services, and events in the area and incorporates into activity schedule as appropriate.
• Establishes a warm and welcoming atmosphere for residents and staff.
• Promotes teamwork and positive attitude among the staff and monitors/attends activities with other disciplines as necessary.
• Continually evaluates the quality and consistency of activities for residents and makes adjustments, corrections, and changes as necessary.

Overview:

The Dining Services Director leads the execution of the signature dining services program at the community. He or she provides friendly, responsive service to create an exceptional dining experience for all of our residents. Each Dining Services Director’s primary objective is to exhibit the core values of Commonwealth Assisted Living and provide a memorable dining experience for the residents.

Responsibilities:

• Prepare and serve nutritious meals to our residents in a safe sanitary manner in accordance with Public Health Regulations, and in compliance with Sanitation Code.
• Prepare food for regular and therapeutic diets according to the planned menu and written doctors’ orders.
• Ensure that the proper quantity of food is prepared and in accordance with established time schedules.
• Prepare food for special events as instructed by Director.
• Maintain costs within budgeted parameters.
• Use proper technique to taste prepared food to ensure quality taste and palatability prior to service.
• Maintain the proper temperature of food during preparation and service.  Record temperatures of food according to established policy.
• Report all hazardous and malfunctioning equipment to the Director.
• Make recommendations to the Director regarding improvements in recipes, menus, productions and service of food, equipment, etc. to enhance the quality of the food service.
• Attend and participate in workshops, seminars and in-services, to keep abreast of current changes in the health care field, as well as maintain a professional status.
• Assist in scheduling department working hours, personnel, work assignments, etc. to maintain quality resident care.
• Routinely inspect the dietary area and practices for compliance with current applicable regulations and as requested by the Director.
• Develop and utilize comprehensive inventory control procedures.
• Purchase food, supplies, and equipment, as required to meet the needs of the department.
• Ensure that a stock of staple/non-staple food, supplies, equipment, etc., is maintained at adequate levels at all times to perform departmental functions.
• Ensure that there is always a 72-hour emergency food and drinking water supply per regulations.
• Maintain confidentiality of all pertinent resident care information to ensure resident rights are protected.

Overview:

The overall purpose of the Maintenance Director position is to plan, direct and control the overall maintenance of the community’s physical plant. This position must perform or oversee electrical, plumbing, carpentry, heating, ventilation, air conditioning, refrigeration and other technical tasks, as well as ensuring compliance with all local, state, and federal life-safety code regulations.

Responsibilities:

• Coordinate and assure performance of all preventative maintenance schedules.
• Visually inspect all common areas on a daily basis to ascertain maintenance needs.
• Prepare work schedules and follow up for maintenance personnel by managing and delegating the work orders and maintaining schedules.
• Set up periodic inspections of the total property systems of checking such items as roof, building façade, parking lots, grounds, signs, etc.
• Initiate, supervise and maintain an up-to-date repair record on all equipment and maintain an up-to-date work list for the job and ensure all work is completed in a timely and effective manner with respect to assigned priorities.
• Initiate a training program to insure training is implemented and completed by all maintenance personnel, inspects all fire-fighting equipment according to time period and instruction required by law.
• Operate and maintain all buildings and grounds, all security and transportation systems in accordance with Federal, State and local OSHA standards.
• Inspect vacated apartments and complete the painting, repair and general refurbishment within the prescribed time frame.
• Head up the Safety Committee.
• Respond to maintenance emergencies at any time needed.
• Maintain grounds and outside areas of building, including and not limited to: sprinkler system, outside lights, driveway and parking lots, in a clean and working condition.
• Continuing education regarding maintenance methods, chemicals and standards.

Overview:

The Cook, under the direction of designated management and supervisors, will prepare, present, store, and dispatch all food products, specifically, but not limited to Dining Room, Bistro (where located), marketing and catering events and resident tray service where required. This will include various functions to pre-set menu specifications within the given time limitations as previously determined by the Executive Chef and Guest, according to the standards of Commonwealth Assisted Living as determined by the Executive Chef, and in compliance with all health and safety regulations by ensuring adherence to all sanitary and safe food handling guidelines at all times.

Responsibilities:

• Professional diploma or food services management experience required.
• Minimum 1 year of cooking experience required.
• Working experience in the retirement home industry preferred.
• Demonstrated knowledge of food and catering trends, quality, production, sanitation, food cost controls, and presentation required.
• Knowledge of methods and procedures for serving food, principles of sanitation, and principles of safe food handling required.
• Ability to lift up to 50lb required.

• Be familiar with Hazardous Analysis Critical Control Point {HACCP}, Workplace Hazardous Material Information System {WHMIS}.
• Responsible for ensuring a clean, safe working environment and maintaining this responsibility by safe work habits, using correct hand washing techniques and routine cleaning.
• Serve attractive, nutritious meals in a pleasant, respectful manner, mindful of the special needs of the residents.
• Practice safe usage of all equipment within the department and the ability to clean, sanitize and maintain the equipment as outlined in the cleaning schedules.
• Prepare the dining rooms daily:  set tables, pour beverages, clear and clean dishes.
• Upon receiving stock, put it away following the procedures as required.
• Follow cleaning procedures when mechanical equipment is not available {ex: power failure}.

Overview:

The housekeeper is responsible for maintaining a clean, safe, and comfortable environment in all areas of the community, as directed by the Maintenance Director, and in accordance with current federal, state and local standards, guidelines and regulations governing the community.

Responsibilities:

• Perform day-to-day housekeeping functions with established procedures as assigned.
• Follow established safety precautions when performing tasks and when using equipment and supplies.
• Clean floors, to include sweeping, dusting, damp/wet mopping, stripping, waxing, buffing and disinfecting.
• Clean carpets, to include vacuuming, shampooing, deodorizing, and disinfecting.
• Clean and polish any furnishings in hallways and, if applicable, in stairways and elevator.
• Clean resident rooms, to include vacuuming, mopping, dusting disinfecting, and sanitizing bathrooms, cleaning mirrors and pictures.
• Collect all trash from rooms, offices, lounges, and bathrooms daily.
• Clean walls, windows and baseboards by washing, wiping off, dusting, spot cleaning, disinfecting, deodorizing, etc.  Keep all windows, cabinets, doors, and mirrors smudge-free and clean.
• Assist in setting up dining room, activity room, and living room as necessary.
• Report any needed repairs to maintenance personnel or Administrator.
• Keep all housekeeping equipment, storage areas, janitor’s closets, and housekeeping carts clean and uncluttered, and free of any hazardous equipment and supplies, etc.
• Respond to residents’ needs as requested.
• Launder community linens as needed.
• Inform Maintenance Director of all supply needs in a timely manner.
• Report all accidents/ incidents to the Administrator on the day of occurrence, along with important resident observations.
• Ensure that cleaning schedules are followed as closely as possible.
• Maintain confidentiality of all pertinent resident information to ensure resident rights are protected; honor residents’ personal property rights.

• Qualifications:
• Possesses a minimum of one year experience in a Certified Nursing Assistant role for an Assisted Living, Independent Living, Skilled Nursing Facility or Memory Care community.
• Experience completing daily shift reports and communicating directly to the next shift any urgent needs.
• Prior responsibility for daily documentation in resident records including task sheets and demonstrated ability to manage assigned tasks within the scheduled shift
• The ability to meet the physical and emotional needs of the resident.

Responsibilities:

• Administers and accurately records the administration of medications for residents as prescribed by the physician in accordance with established policies and procedures of Commonwealth Assisted Living and applicable state/federal regulations. Observes, records and reports to the Resident Care Director, or designee, symptoms and conditions of residents that could be related to medication interactions, adverse drug reactions or medication side effects.
• Determines and records effectiveness of medications in a timely and accurate manner as required by state and federal regulations and provides and promotes resident’s rights during the administration of medications.
• Stores medications in a safe and accurate manner in accordance with established policies and procedures of Commonwealth Assisted Living and applicable state/federal regulations.

Overview:

The Program Director plans, supervises and monitors the “Sweet Memories” and Assisted Living programs to meet the specific needs of each and all residents in order to ensure the residents remain as engaged, healthy and as active as possible. Assists the Resident Care Director in maintaining the physical and emotional health of the resident.  The Program Director is responsible for the activities program in the memory care neighborhood and assisted living.

Responsibilities:

• Designs a creative activity program based on company standards to meet the individual needs and interests of the residents. Plans seven-day per week activity calendar (s) that promotes resident participation in a structured environment; includes one to one programming for residents who cannot participate in a group setting.
• Work closely with the Resident Care Director on a daily basis to better serve the residents, family, visitors and staff in the community.
• Monitors the day-to-day operation of the Sweet Memories Neighborhood, i.e. resident ADLs, meals, cleanliness of the neighborhood, delivering of activities, charting and documentation
• Follow state-mandated Assisted Living Facility
• Participate in the delivery our CAL Signature Service Programs to Sweet Memories and Assisted Living residents.
• Assist and provide input in the completion and updating of UAIs, ISPs, and other assessments specific to the Sweet Memories Program
• Participate in Care Plan meetings and family meetings.
• Communicates to families regarding resident’s involvement in activity program.
• Assist as needed in the training and teaching of staff for the Sweet Memories Neighborhood and Assisted Living activity programs.
• Provides guidance and acts as a resource to staff on the importance of activity programs and other areas regarding the quality of life for residents.
• Participate in resident move-in processes in the “Sweet Memories” neighborhood and Assisted Living.
• Assists in touring and customer contact to maintain desired occupancy.
• Participates in coordinating special events at the community to include family nights and open houses
• Manages volunteer program, including training and supervising volunteers.
• Weekend coverage as assigned
• Be able to do any job in the facility upon request by the Executive Director
• Directly supervises any Activity Assistants in all duties and responsibilities to include interviewing, planning and, assigning, and directing work; appraising performance; addressing complaints and resolving problems.
