Key Responsibilities

• To assist in the collection and sorting of soiled personal clothing and linens and pre- treat stains as required;
• To load and unload washers and dryers ensuring that proper settings are used and that chemical supplies are at an appropriate level;
• To inspect soiled linens or personal laundry and complete any required repairs, e.g. rips, tears, buttons;
• To fold , segregate and stack linens and residents' clothing and ironing as required;
• To deliver linens and clothing to appropriate areas;
• To adhere to established facility policies and procedures, including safety rules and safe working practices;
• To complete documentation as required according to facility policy/job routines;
• To be aware of the Resident Rights & Responsibilities and endeavor to respect and promote it;
• To attend in-service training as required in order to better serve the needs of the residents;
• To attend staff meetings as required to be aware of changes in Residence that have taken place;
• To participate in the Quality Improvement Program;
• To uphold and promote the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in Revera’s Code of Conduct;

We are looking for a Personal Support Worker who has their current PSW Certificate, availability to work days, evenings, nights, weekends, has the ability to work independently as well as part of a team, well-developed interpersonal skills, excellent organizational skills, effective problem-solving and conflict management skills, ability to work well within a changing environment, demonstrated appreciation for customer service, and current CPR certificate. Must be able to lift, transfer and assist residents.

We are looking for a Registered Massage Therapist with 1 or more years of experience and applied knowledge of the senior population. The successful candidate will have competency in various types of massage techniques combined with exceptional customer service skills. Must have your own insurance and a vulnerable sector police check.

Reporting to the Director of Health & Wellness, the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

• To work as a member of the inter-professional team as described by Revera’s Professional Practice Model applying the person centered approach to care;
• To complete and ensure resident’s clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required;
• To implement and evaluate the effectiveness of the care/service plans and make changes as required;
• To assist with scheduling of resident’s care conferences and participate in conferences where applicable;
• To work closely with the Director of Health & Wellness to facilitate resident’s move in, move out and transfers;
• To provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Revera Resident Care standards and policies and procedures;
• To process the orders and safe keep drugs and narcotics and administer in compliance with all regulatory requirements, scope of practice and Revera policies and procedures;
• To facilitate and provide resident/family education;
• To plan, coordinate, manage & evaluate the resident’s care plan working with the resident/substitute decision maker, physicians, nursing & interdisciplinary team and contracted service providers;
• To coordinate nursing team’s resident care assignments to ensure efficient and timely delivery of services to residents;
• To respond to and manage significant changes in resident’s condition and or medical emergencies; provide emergency first aid;
• To teach, delegate to, and direct unregulated care providers;
• To coordinate emergency response in the absence of Director of Health & Wellness/ Executive Director;
• To respond to resident/family concerns and ensure appropriate action taken and or concern communicated to Director of Health & Wellness/ Executive Director and other departments;
• To follow and assist the Director of Health & Wellness in the coordination, implementation and monitoring of Infection Prevention and Control , Health and Safety and Clinical Programs and
• Quality/Safety Plans for the Nursing Department;

Reporting to the Nursing Supervisor, the RN is responsible for the assessment and provision of professional nursing care to clients in the community setting.

• To assess clients’ status and develop/ revise care plans and treatment regimes , including health teaching accordingly;
• To access clients’ safety status, including physical environment, make recommendations, implement interventions and advocate on client’s behalf to ensure client’s safety is maintained;
• To document clearly and accurately using the appropriate Revera forms in accordance with CNO standards;
• To maintain client confidentiality ;
• To provide professional nursing care in accordance with regulatory body standards& guidelines, applicable legislation and Revera’s policies and processes;
• To participate in clinical case conferences as required;
• To maintain competency in nursing skills;
• To communicate appropriate client information within the client’s interdisciplinary team to promote quality, client focused care;
• To communicate any serious client or risk concerns immediately to the Nursing Supervisor;
• To maintain an effective working relationship with all internal and external stakeholders;
• To mentor new staff/ students as required;
• To participate in in-service education;

Reporting to the Director of Health and Wellness (DHW) / Director of Care (DOC), the Registered Nurse (RN) is responsible to provide direct resident care and medication administration and document care in compliance with regulatory and Revera Resident Health and Wellness standards and policies and procedures.

• To complete and ensure residents’ care assessments, service plans and documentation are up to date and changes communicated to DHW/DOC for billings;
• To assist with scheduling of residents’ care conferences and participate in conferences;
• To work closely with DHW/DOC to facilitate residents’ move in and transfers;
