Position Summary: Enhances resident services and provides up to date information in a more efficient and timely manner. Greets residents and visitors, answers the telephone and responds to both routine inquiries and emergencies, relays calls to the proper department and performs a variety of related clerical duties. This position reports to the Business Office Manager or other assigned manager.

Essential Functions:

• Greets residents and provides general assistance to visitors, prospective residents, and families.
• Uses proper telephone skills.
• Ensures safety of the residents and property by monitoring surveillance cameras, door alarms, life safety systems, building alarms and investigating any unusual activity.
• Ensures that incident reports, concern reports and other appropriate communication have been completed.
• Maintains visitor sign in/out book, reservation book for guest rooms, and completes maintenance request work orders.
• Completes daily check-in procedure.
• Maintains emergency contact information.
• Receives packages, plants, prescription drugs, etc. and delivers/holds as requested.
• Maintains the sign in/out log for the issuance of community keys, and gate passes.
• Informs supervisor of any resident issues or concerns.
• Maintain coffee and cookie service in the lobby/making cookies daily.
• Makes sure the lobby has fresh flowers and is appropriately appointed.
• Assist other departments with administrative functions.
• Completes all required daily shift tasks as per community established by the ED up to and including rolling over the phones at a consistent time daily.
• Attends all required training, in-service, and staff meetings.
• Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
• Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
• Adheres to all policies and procedures of Senior Lifestyle Corporation.
• Performs other duties as assigned.

Reporting to the Executive Director of the home, the Director of Care is responsible for planning, directing, coordinating and supervising the overall nursing program for residents within the home. This position is based in Sherwood Court in Maple, Ontario.

• To facilitate and promote the interdisciplinary approach within the home through the Nursing, Dietary, Activation, Environmental Service and Administrative Departments
• To review all resident applications for admission to determine appropriate level of care; assist with processing of admission and discharge papers
• To implement and monitor control and safekeeping of drugs and narcotics supervise the drug administration program including the administration of medication by all registered staff
• To direct and monitor the maintenance of health care records to ensure compliance with regulations and quality care for the resident
• To participate in Care Conferences as appropriate, meet with residents/family to resolve problems/ concerns/complaints
• To assist the Executive Director by collaborating with the Medical Advisor to ensure that medical attention and records are in compliance with regulations
• To hire, orient, schedule To assist the Executive Director in ensuring staff are knowledgeable about Resident Bill of Rights and Health and Safety Standards
• To monitor and review resident care staff compliance with all facility policies and procedures, including safety rules and safe working practices
• To comply with all relevant Provincial and Federal Acts and Regulations
• To assist the Executive Director with budget preparation and implementation for the Nursing Department
• To participate in the review process by regulatory bodies
• To investigate complaints
• To co-ordinate and monitor the Infection Control Program for the Nursing Department
• To assume administrative duties as required in the ED’s absence
• To uphold and promote the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in Revera’s Code of Conduct
• To complete all other duties as assigned
• Supervise and evaluate the performance of nursing personnel; including use of disciplinary process in conjunction with Executive Director.

Reporting to the Nursing Supervisor, the RN is responsible for the assessment and provision of professional nursing care to clients in the community setting.

• To assess clients’ status and develop/ revise care plans and treatment regimes , including health teaching accordingly;
• To access clients’ safety status, including physical environment, make recommendations, implement interventions and advocate on client’s behalf to ensure client’s safety is maintained;
• To document clearly and accurately using the appropriate Revera forms in accordance with CNO standards;
• To maintain client confidentiality ;
• To provide professional nursing care in accordance with regulatory body standards& guidelines, applicable legislation and Revera’s policies and processes;
• To participate in clinical case conferences as required;
• To maintain competency in nursing skills;
• To communicate appropriate client information within the client’s interdisciplinary team to promote quality, client focused care;
• To communicate any serious client or risk concerns immediately to the Nursing Supervisor;
• To maintain an effective working relationship with all internal and external stakeholders;
• To mentor new staff/ students as required;
• To participate in in-service education;
• To uphold and promote the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in Revera’s Code of Conduct;
• To complete all other tasks as assigned.

Reporting to the Client Services Manager, the Social Worker maximizes the client/family’s functioning within the context of rehabilitation, maintenance or palliation. The Social Worker enhances the problem solving and coping capacity of individuals and families/caregivers utilizing a variety of interventions.

Key Responsibilities

• To evaluate Client need based on presenting problems employing standardized assessment tools
• To assess the safety and risk factors of the client/caregivers/health care team and set out appropriate resolutions and/or coping strategies
• To determine whether Social Work is required
• To design a treatment plan considering evidence-based information, relevant best practice guidelines and clinical expertise
• To set specific, measurable, achievable, realistic, time-limited and client centered goals
• To determine the need for additional or alternative services
• To organize workload considering client and other team members’ needs
• To incorporate discharge planning into treatment planning/process
• To select and execute appropriate, cost effective intervention strategies
• To educate Clients, Families/Caregivers and the health care team regarding the nature of the client’s illness/ problem, purpose of the interventions, and self-management strategies
• To link Families/ Clients with appropriate community partners/ services
• To evaluate the effectiveness of the intervention, the client’s response, and progress toward goal attainment and/ or need for changes in service
• To evaluate client outcome using appropriate outcome measures
• To collaborate and communicate with the Health Care Team regarding assessment findings, possible barriers, treatment and future planning
• To participate in case conferences as required
• To maintain daily and weekly records of client care activity, which comply with Revera’s regulatory body standards
• To submit timely and accurate clinical and administrative reports, including billing and payroll invoices

The Recreation Attendant is accountable for providing recreation and leisure activities as scheduled and directed by a Recreation Therapist. The Recreation Attendant will work to ensure the continued organization and smooth running of new and ongoing programs and activities in accordance with Bethany Care Society’s philosophy, objectives and standards. Driving resident passenger buses required.

Bethany’s continuing care programs provide around–the–clock professional care and support for people with complex health needs who are unable to remain at home or in a supportive living environment. In this position, you will provide support to three separate Bethany sites; Bethany Airdrie (40% or 2 days per week), Bethany Cochrane (40% or 2 days per week) and Bethany Harvest Hills (20% or 1 day per week). You will be accountable for the planning, implementation and administration of programs designed to enhance the nutritional care and well-being of the residents in a safe and effective manner. Your responsibilities will include:

• Assessing the nutritional status of residents and developing individual nutrition care plans in accordance with resident and family goals
• Documenting and communicating nutrition care plans
• Reviewing and evaluating nutrition care annually or at the request of resident, family, or interdisciplinary team members.
• Liaising and maintaining effective communication regarding resident nutritional care with the care team, resident, family and food services staff
• Participating in comprehensive auditing activities and ensuring clinical standards are met.
